Each of our Sub-Acute and Rehabilitation Facilities provides a sub-acute level of care to patients who are medically stable and no longer require high intensity acute care services.
We focus on enabling patients to regain functional independence through skilled nursing care and rehabilitative services. Most patients come directly from a short-term care hospital or are transferred from our long-term acute care hospital. Our patient-oriented staff members work toward successfully transitioning patients to a less intensive level of care service, such as a skilled nursing facility or home.
Our Washington, D.C., locations offer LTACH, sub-acute and rehabilitation, and SNF services in the same building. This creates easier transitions between levels of care if needed.
